Cardiovascular disease in pregnancy

mWHO 2.0 — maternal cardiovascular risk

Classifies maternal cardiovascular risk with the modified WHO 2.0 classification from the 2025 ESC pregnancy guidelines. That version adds cardiomyopathies, channelopathies, Ebstein anomaly, arterial switch, prior SCAD or ACS, prior adverse pregnancy outcomes and cancer-therapy cardiotoxicity. The highest class among the patient's conditions applies. It sets Pregnancy Heart Team involvement (mandatory from class II–III), counselling, care during pregnancy and place of delivery. Class IV calls for a shared decision with the woman, replacing the earlier blanket advice against pregnancy. It also adds contraception, genetics, aortic imaging and mechanical-valve anticoagulation advice.
