Cardiovascular disease in pregnancy

Hypertension in pregnancy and pre-eclampsia: algorithm

Classifies the hypertensive disorders of pregnancy from the gestation, BP, platelets, liver enzymes, creatinine and the clinical and fetal findings, using the ISSHP 2021 definitions and the ACOG severe features. It then gives the steps: emergency treatment of BP ≥ 160/110 within 30–60 min, magnesium sulphate for severe features, HELLP or eclampsia, fluids and labs, oral treatment from 140/90, low-dose aspirin for prevention, and the timing and mode of delivery by gestation (37 weeks without severe features, 34 weeks with them, at any gestation after stabilisation for eclampsia, HELLP or deterioration). Postpartum follow-up, breastfeeding-compatible drugs and the lifelong cardiovascular risk are included. ESC thresholds sit beside ACOG, ISSHP and NICE.
