Mitral TEER: indications and echo suitability

Decides whether mitral transcatheter edge-to-edge repair is indicated in ventricular secondary, atrial secondary or primary MR. For ventricular secondary MR it checks optimised FMT and CRT first, then the criteria that predict benefit (LVEF 20–50%, LVESD ≤ 70 mm, systolic PA pressure ≤ 70 mmHg, no significant RV dysfunction, haemodynamic stability) and MR proportionality (EROA/LVEDV). It then grades the echo anatomy for TEER: valve area, gradient, posterior leaflet length, coaptation depth and length, flail gap and width, jet origin, calcification and leaflet pathology. ESC and ACC/AHA recommendations are shown side by side.

Mechanism and severity
Severity on TTE, confirmed with TOE when needed

In secondary MR an elliptical orifice or low flow can underestimate severity: EROA ≥ 0.3 cm² or RVol ≥ 45 mL may already be severe.
