TAVI suitability — Heart Team guide

Grades aortic stenosis from echo (including low-flow, low-gradient AS confirmed by CT calcium score or dobutamine stress echo), sets the indication for intervention, then weighs age, surgical risk (EuroSCORE II, calculated here, or STS-PROM), frailty (Essential Frailty Toolset) and CT findings — valve morphology, annulus size, coronary height, LVOT calcium and iliofemoral access — to suggest transfemoral TAVI, SAVR or an individualised Heart Team decision.

Patient
Echocardiography
Severity grading
CT (TAVI planning)
ECG-gated CT angiography of the aortic root and iliofemoral axis
LVOT / annular calcification
Concomitant disease
Lesions better treated at surgery; also sets the EuroSCORE II weight of intervention
EuroSCORE II
Surgical mortality risk. Uses age, sex and LVEF from above.
NYHA class
Creatinine clearance (Cockcroft-Gault)
PA systolic pressure
Urgency
Essential Frailty Toolset
Predicts 1-year mortality after TAVI or SAVR (FRAILTY-AVR)
Five chair rises without using arms
